Until recently, the burden of safety was put on the worker, with urgings to be careful. Most experts now say that, in a dangerous occupation, even the most cautious operator runs too high a risk. Many bosses ignore safety problems because they see the costs of workers’ protection cutting into profits.

In Los Angeles, former District Attorney Ira Reiner gave bosses of unsafe operations a new motivation to make their places safe: he arranged for their prosecution and trial; some even were sent to prison. Twenty-four hours a day, 7 days a week, Mr. Reiner had a lawyer and an investigator standing by, ready to respond to the report of a death on the job.

«When a worker is killed, fellow workers are appalled and upset and will speak candidly to investigators immediately after the accident,» Mr. Reiner explained. «But if you wait a few days, things change. The employer talks to the other workers, who see their jobs at stake.

«By then, an entirely different attitude has set in. If there is a deliberate violation of the law, the highest company official who had guilty knowledge will be personally charged. We are now getting phone calls from lawyers at corporations, asking our advice on how to set up safety programs.»

In July 1987, California Governor George Deukmejian killed the state’s office of the Occupational Safety and Health Administration (CAL-OSHA). He turned inspection and regulation over to the Federal OSHA. Last November, voters restored the state’s jurisdiction to inspect private-sector workplaces.

John Lynch, who once headed Ira Reiner’s occupational crimes unit says, «I have seen a dramatic increase in fatalities since the feds took over.» The federal agency rarely refers cases for prosecution. Instead, the agency relies on «mega-fines,» penalties that can total more than $1 million.

John Pendergrass, of OSHA, says that his is a civil, not a criminal agency. «We want the employer to make the workplace safe in a timely manner,» he says. So OSHA imposes the heavy fines, which can increase as time elapses, to motivate employers to act promptly.

Joseph Kinney of the National Safe Workplace Institute says that OSHA often ends up cutting its fines by 50 percent or more, taking the bite out of them. Mr. Pendergrass says OSHA’s approach obviously works because the on-the-job death rate is dropping. This, counters Mr. Kinney, is a mirage due in part to a shift in employment – from risky manufacturing, mining, and farming jobs to jobs in the far less hazardous service industries, including retail trades, repairs, and offices.

Moreover, Mr. Kinney says, in the last 8 years, owing mainly to OSHA’s weak approach to job safety, the decline in the death rate has slowed from 2.2 percent annually to only 0.7 percent. If the reduction had continued at the old 2.2 percent rate, Mr. Kinney says, nearly 4,000 fewer deaths would have occurred in the last 4 years. Pendergrass calls this «statistical manipulation.»

The records show that OSHA issued 1,238 citations for «willful» violations (the gravest of its offense categories) in 1980 but only 523 in 1981, the first year of the Reagan Administration. Last year’s total was 1,159. In addition, OSHA cut its inspectors from 1,350 in 1980 to 1,150 in 1981.

Mr. Pendergrass says OSHA is doing better with fewer inspectors and fewer citations. But the experts we talked with say they doubt this. They also criticize him for not setting priorities to go after dangerous industries, and they say he has not established safety standards in a timely way. It takes 2 to 6 years for a regulation to get on the books. And hundreds of industrial chemicals are so new that no rules have been formulated yet to regulate their use or storage.

ILLNESS CAUSED BY FOOD: CHEMICAL POISONING AND RADIOACTIVE FALLOUT

Chemical poisoning

Food may be accidentally contaminated by poisonous chemicals. Lead poisoning can occur if food is exposed to dust containing lead or if food is kept in containers made with solders, alloys, or enamel containing lead.

Zinc, an essential mineral element, is toxic in excessive amounts. Illness was reported in a group of people who drank lemonade made in a galvanized can. The acid of the lemon juice had dissolved out zinc from the can. The discharge of mercury into rivers and oceans can lead to fish contaminated with greater than tolerable limits; poisoning from this source was recently reported in Japan.

Pesticides have sometimes been accidentally mistaken for a food ingredient, or have been ingested by children who had access to them. Pesticides used in excess of regulations are a potential hazard to fruits and vegetables. Careful washing of foods before their use reduces this hazard.

Radioactive fallout

Nuclear testing has increased the amount of strontium 90 and iodine 131 in the atmosphere and consequently in the soil. Cattle may transmit strontium 90 from the grasses they eat to their milk, and plants grown on such soils may also contain radioactive elements. The absorption of large quantities of iodine 131 by the body increases the possibility of thyroid cancer. About four fifths of the strontium 90 is excreted in the urine, but some is deposited in the bones and gonads. A liberal intake of calcium appears to be protective against excessive deposit of strontium 90. The Atomic Energy Commission and the U.S. Public Health Service measure the amounts of radioactive fallout in food supplies from time to time. Presently, the amounts are well below any danger levels.

CHILD’S HEALTH/SKIN DISORDERS: LICE (PEDICULOSIS) TRETMENT AND PREVENTION

Treatment

1. Anti-lice shampoos are available from the chemist without a prescription. (They contain either malathion or pyriform as the active ingredient.) Pour the recommended amount onto dry hair and add enough warm water to form a lather. Shampoo the scalp and hair for several minutes and rinse thoroughly. Repeat this process in another week to ensure that both lice and eggs have been destroyed. This treatment is safe and effective, but if lice recur it may mean that your child has been in contact with an infected person again.

2. Comb out nits carefully with a fine-toothed comb. You do not need to shave your child’s hair, but an ordinary haircut may help a little.

When to see your doctor

• if your child is itching excessively and it interrupts his sleep;

• if sores develop on the scalp, and weep or spread.

Prevention

Lice are highly contagious, and can spread between families and between children in the same class. Check the whole family for lice, and even if you do not see any eggs or lice, it may well be worthwhile to treat other family members if they complain of an itchy scalp. It is important to let the child’s creche, kindergarten or school know that your child has lice. Keep your child at home until you have treated him with the shampoo. Clean and vacuum your child’s room, as well as any areas in which he has been playing. Soak all brushes and combs in anti-lice shampoo for several hours to disinfect them. Wash all bedding, blankets and clothes in hot water, and seal hats or other items which can’t be washed easily in plastic bags for 2-3 weeks in order to kill the eggs.

Feelings of not being able to cope are common to many parents, irrespective of age, experience, support systems, or feelings about the pregnancy. They mainly affect the primary care-giver, who is often the mother. There is considerable physical stress during the birth and afterwards — the physical tiredness is real, not imagined. Inevitably, the reality of constant feeding, dirty nappies and interrupted sleep contrasts dramatically with the blissful fantasy you had before the baby was brought home. As parents, you realise that caring for a baby is an enormous added responsibility — as the mother, you often still have to attend to the needs of other family members. In hospital the baby was nursed and your many needs taken care of. At home you may find that you have little or no support and that as well as caring for the baby, you still have a household to run. You may also need to return to work soon after the birth which can add to the stress.

Multiple births

One baby is hard work. Managing two or more babies at the same time can be very difficult for a couple. If you are expecting more than one baby, make sure that you organise plenty of support in advance of the birth. When the babies arrive, call on relatives, neighbours and friends to share in the sheer physical work involved in looking after more than one baby. Seek out other parents of children from multiple births, who may give you great hints, and some much needed understanding. Your local council or maternal and child health nurse will also be able to advise on what home-help and support services are available in your area.

Send and receive messages with an emphasis on learning and listening, an emphasis on watching your partner while he or she talks, on trying to feel what the other is feeling while he or she is talking. «Try again,» I asked the couple. «This time, really watch each other and try to feel what is said, not just hear it.» «Okay,» said the wife. «I really, really want a new couch, just a new couch.» The husband reiterated, «I hear you saying that you really want a new couch.» He looked at me and shook his head. «Now what? She wants a couch. That’s no big deal.»

The husband was wrong. His wife said something, he heard it. That’s as much a start for learning good sex as not hearing a simple message is a start toward bad sex. The trick is to employ the L.O.V.E. sig when it counts, when important issues are being discussed. Always remember to listen, but also to watch each other, verify before responding, and emphasize empathy, trying to get the feeling, not just the words. Listen for each other, not from each other’s personal biases. Much more intimacy can result. It all starts with listening.

SCABIES – DIAGNOSIS

Secondary infection with bacteria can occur and have the appearance of impetigo or school sores, with weeping and crusting.

Scaly patches which look like eczema may also develop.

The scabies mite may cause such a rash and this may not be easily diagnosed. As scabies was rarely seen for many years, many doctors had never seen a case. Now it has become so common, other itchy rashes may be mistakingly thought to be scabies and treated as such.

Finding and identifying a burrow confirms the diagnosis. Removing the mite and examining it under the microscope provides positive proof.

Some sufferers may have the rash and the itch for several years.

Once a correct diagnosis is made, the correct treatment can be applied. The various preparations available are all cosmetically acceptable and most people are prepared to carry out the treatment.

Benzyl benzoate lotion and gamma benzene hex-achloride are the most effective chemicals, just as they are for pediculosis or louse infestation.

ANTI-DEPRESSANTS – TREATMENT

The foods to avoid are banana skins, broad beans, cheese (particularly mature cheese), some red wines, beer, mature protein foods such as fish and meat which are cured and meat, vegetable and yeast extracts, chocolate, yogurt, sour cream, strawberries and avocado.

Stimulant drugs such as adrenalin, present in the local anaesthetic used by dentists, and the bronchodilator drugs used in treating asthma, also need to be avoided.

The more commonly used tricyclic anti-depressants do not have this reaction and dietary control is unnecessary.

I remember treating a woman for depression with one of these agents. She complained of a severe headache, and had all the symptoms and some signs of a sub-arachnoid haemorrhage — a type of bleeding around the brain. I arranged for a specialist to see her and she was thoroughly examined. We were puzzled by the negative findings.

That same week we received a report from the makers of the drug stating that this side effect had been reported by research workers after the drug had been widely used.

I questioned my patient again and found that she had had a large meal of macaroni cheese on the evening she developed the symptoms.
